The patients are accepted for Respective Speciality Centers for short/long term residential care based on the recommendations of the psychiatrist, who has been treating him/her with the consent of family and the person needing such care. Request for admission should be made by the parents / guardians / person concerned in the prescribed application form, obtainable from the office of the Administrator. On receipt of completed application form, the request will be reviewed by the Admission Committee. On approval of the committee the patient will be admitted as short or long term resident, as the case may be, after fulfilling the other terms and conditions stipulated by the CADABAM'S.

A thorough psychological and physical check up of the patient shall be made at the time of admission.

Psycho-social Rehabilitation & Care
General
The CADABAM'S provides short term and long term professional care for the Mentally ill, Deaddiction, halfway house Fecility for Mentally ill & short and long term Habilitation & Rehabilitation for Mentally Retarded adults (male and female)who are in need of Psycho-social Residential Rehabilitative facilities in a institution.
Eligibility, Procedure and Conditions for Admission
1. Any adult (male & female) or Parents/Guardians on behalf of their wards requiring Psycho-social care may seek admission with Referral from a Psychiatrist.

2. Application for admission should be made in the prescribed form, complete in all respects.

3. Admissions shall be subject to payment of prescribed fees/deposits and approval of management.

4. On admission the Resident and Parents/Guardians should abide by the Rules and Regulations of the centre from time to time and as contained in the document of admission.

5. The parents/guardians shall be responsible for any damage, breakage, accidents, injuries, caused by the Resident to the materials and property of the CADABAM'S and to the personnel and other co-residents of the CADABAM'S.

6. The CADABAM'S is only a Care-taking and Rehabilitation centre. The Resident is accepted for this purpose only.

7. Any civil or criminal liability inviting legal action on the Resident existing before admission or arising later shall not be the responsibility of the CADABAM'S.

8. If the Resident requires hospitalization due to pre-existing or prolonged/serious illness or surgery/specialist consultation/investigation etc the same shall be arranged by the CADABAM'S. In such cases all expenses arising out of the additional medical care shall be met by the Parent/Guardian. Except in case of emergency, as far as possible prior information will be given. In the alternative the families will be at liberty to arrange for treatment in a hospital of their choice, with the concurrence of the centre.

9. The care taking fee or deposit paid, covers only food, accommodation, counselling Rehabilitation Therapy/activities, general services, common facilities and not Medical and any special Services like personal attendant, visits/drives outside the campus, picnics, etc.,

10. ALLOTMENT of ACCOMODATION
Allotment of shared or single accomodation is subject to availability and shall also depend on the physical and mental health of the prospective Resident as revealed in the medical reports and history. The preference of the family will also be given due consideration. But the prime consideration shall be the safety and well being of the resident as well as other Residents of the CADABAM'S.

11. VISITS & PHONE CALLS
Parents/Guardians should periodically visit the CADABAM'S to look to the welfare of their ward and also have discussions with the doctors and counselors. The visits shall be between 10.00a.m. and 4.00p.m. The telephone calls to counsellors/special Educator should be between 2.00 to 3.00p.m. except on sundays. To meet Secretary or Medical Director or Director or Administrator prior appointment is advisable. The parents/guardians/family members can call the Residents on Phone between 1.30p.m and 3.00p.m and talk to them. Such calls shall be restricted to 3-5 minutes only and the calls should not be too frequent and can be spaced at intervals of 15days to a month. This restriction is in the interest of the Resident as frequent calls may disturb the Resident. The counsellors will be at liberty either to call or not to call the Resident depending on the state of Mental Health of the Resident.

12. LEAVE OF ABSENCE (SHORT AND LONG PERIODS)
Residents can be taken out of the home with the approval of the Medical Director and Counsellor/ Administrator/Director and their decision shall be final. The duration shall be as per their decision.

13. SMOKING
Smoking shall be regulated as may be advised by the Medical Director/Management.

14. CONSUMPTION OF ALCOHOL. Etc.,
No intoxicating drink/narcotics/ghutka and other habit forming substances shall be permitted to be kept with the Resident nor consumed, while in Residence. Families should ensure that the Residents will have no access to such substances while they are taken out of the centre on leave of absence.

15. POSSESSION OF CASH, CELL PHONES, VALUABLES LIKE GOLD, MUSIC SYSTEMS, CD PLAYERS Etc.,
The Residents will not be permitted to keep with them Cell Phones, Cash or Valuable Ornaments made of Gold, Precious Stones. Women may be permitted to wear traditional ornaments like Ear-rings/Bangles/ Necklaces/Mangalsutra, etc., provided the family gives a declaration that they will not hold the centre responsible for any loss of such things. It is desirable not to give the resident costly wrist watches. The families can provide Music Systems, CD Players to their wards. Only audio discs are permitted. Use of VCD/DVD discs are prohibited.

16. MEDICAL AND PERSONAL EXPENSES
All medical and personal expenses on toiletries, clothes, etc., which are incurred for the resident shall be billed on monthly basis and should be paid within ten days from the date of billing.

17. PAYMENT SCHEMES
Three types of Payment Packages for both short and long term rehabilitation programmes viz.Monthly, Annual and Lumpsum One time Payment schemes are available to choose. All payments will be receipted. The amount of Payment depends on the type of accomodation provided and facilities sought for and will be covered by the admission document. In the case of monthly and annual payment there will be an increase of 15% in the fees from 1st April each year in the case of monthly payment and after expiry of each year of stay in the case of annual payments.

18. REFUNDS
On discharge/Withdrawal/Death refunds shall be on Pro Rata basis depending on duration of stay only in the case of Lumpsum-one time payment, made for long term rehabilitation. Refunds shall be made within a period of twelve months from the date of discharge. For any delay beyond twelve months interest at 6% P.A. shall be paid.
